Medical Records
Referring doctors will always be sent a summary letter of your visit. In addition, medical records
can be released to other physicians with written authorization from the patient’s legal guardian or from the patient (if the patient is a legal adult). Medical records can be released directly to the patient for a fee.
Records will be sent by mail. We will not provide copies of records from other institutions; you will have to contact those organizations separately for records.
